Miracle Day

VOLUNTEER RELEASE AND CONSENT

Miracle Day is a volunteer program sponsored by The Christian Church (Disciples of Christ) Alabama and Northwest Florida and First Christian Church of Cullman(jointly referred to henceforth as the Sponsors) to perform needed cleaning. maintenance, repair and construction work for congregations and other related entities of he Christian Church (Disciples ofChrist) who may not be able to accomplish this work on their own. Such work may involve hard physical labor, lifting and other strenuous activities, some of which may take place on ladders, scaffolds and other locations above or below ground.

I desire to participate in Miracle Day by volunteering my time and talents. As a condition to my participation I agree to the following:

1. I acknowledge that the work I will perform may be strenuous and could result in injury, and I accept full responsibility for all bodily injury or personal injury which I may sustain, and agree for myself and for my estate and heirs to release, discharge and indemnify and forever hold the Sponsors and the congregation or other organization on whose property I may he working harmless from any and all causes of action and claims which I may, at any time, have as a result of my voluntary participation in Miracle Day. I further acknowledge that any work I perform is strictly voluntary, and that I have the right to refuse to perform and will in fact not perform any work which I believe is beyond my capability or physical ability.

2. I agree that I will abide by any rules and regulations that may be established in conjunction with the Miracle Day project on which I have chosen to volunteer.

3. I acknowledge that on Miracle Day, the Sponsors may make photographic and/or video recordings of all or a portion of any Miracle Day (Photos and Recordings”), which could include my image or likeness and I understand that such Photos and Recordings may be included in video tapes, books, booklets, brochures, displays and electronic media which may he published or otherwise made available to the general public. I hereby consent to such Photos and Recordings and waive any and all rights to compensation for the use of such Photos and Recordings.
